| Abstract: This package provides instructions and forms necessary for States to report the number of written, signed complaints; mediation requests; and hearing requests and the status of these acitons with regards to children being served under Part C of IDEA initiated during the reporting year. The form satisfies reporting requirements and is used by the Office of Special Education Programs (OSEP) to monitor SEAs and for Congressional reporting. |
